Why people use Discharge Summary Simplifier

Indian hospital discharge summaries are written for the next-treating doctor, not the patient. Diagnoses in medical Latin, drugs in clinical shorthand (1-0-1, BD, TDS), follow-up plans assuming clinical context. The simplifier rewrites all of it in plain English the patient and family can act on. This is a language-translation aid, NOT medical advice.

Patient + family understanding
Hospital stays end with a discharge summary the patient is expected to act on — medications, follow-up appointments, warning signs. Plain-English rewrite means the patient (and the family member who’ll administer the medications) actually understand what to do.
Telemedicine consult prep
Follow-up consults with a primary doctor after hospital discharge often need a recap of what happened in hospital. Simplifier output is the recap — better than handing the doctor 8 pages of clinical jargon to skim.
Insurance / TPA reimbursement
Insurance Third Party Administrators need a procedure-and-treatment summary for cashless / reimbursement processing. The simplifier output (with structured medication list and procedure description) accelerates the TPA review.
Caregiver handoff
When the discharge happens during one family member’s shift but recovery happens at home with another, the rewritten summary is the handoff document. Reduces “what did the doctor say about the medicine?” phone calls.
Cross-system specialist consult
Indian patients often see specialists across hospital systems. The plain-language summary is shareable across systems without each specialist needing to decode the originating hospital’s style.

How Discharge Summary Simplifier works

Three steps. Sign in to run, zero retention — files never persisted on our servers.

  1. STEP 1
    Drop the discharge PDF
From any Indian hospital — multi-speciality, smaller nursing home, government hospital. Up to 50 MB.
  2. STEP 2
    We rewrite section-by-section
Diagnoses translated from medical terminology to everyday words (with the original term in parentheses). Medications rewritten with timing in plain English (“Tab Pan-D 40 mg 1-0-0” becomes “Pantoprazole 40 mg, one tablet in the morning before food”). Follow-up plan + warning signs surfaced prominently.
  3. STEP 3
    Get patient-friendly markdown
Output sections: what happened in hospital / current diagnosis in plain English / medications with timing / follow-up plan / warning signs to watch for. Page citations link each section back to the original discharge.

What makes pdfcraft ai different

  • Citations on every claim
Every AI summary, answer, and translation links back to the page it came from. You can verify the source before relying on the output. Hallucination is bounded, not blind.
  • Pay only for what you use
No subscription. Purchased credits never expire. Cancel anytime. Cost shown before you run — exact credit amount appears after upload.
  • Multi-provider routing, no vendor lock-in
We balance multiple AI providers per call to optimise cost and quality. If one has an outage, the next call automatically tries another. You're not bound to a single vendor's pricing or downtime.
  • Zero retention by design
Your PDF lives in memory only as long as the AI call needs it, then it's discarded — nothing is persisted to our servers. We don't fine-tune models on your data. Verifiable in your dashboard's audit log.
  • Honest about model limits
When a doc exceeds the model's context window we surface that the output was truncated (we render result.wasTruncated in every AI tool's UI). When a number can't be verified from the source we flag it. No silent failures.

Frequently asked questions

Is this medical advice?
No. The simplifier translates the LANGUAGE of the discharge summary — we don’t add clinical interpretation, change dosages, or recommend skipping medications. The clinical content stays exactly what the prescribing doctor wrote. Always discuss the discharge with the prescribing doctor if anything is unclear.
How does it handle Indian prescribing shorthand?
Pre-encoded: 1-0-1 (one tablet morning, none afternoon, one tablet night), BD (twice a day), TDS (three times a day), QID (four times), HS (at bedtime), SOS (when needed), AC (before food), PC (after food). All translated into plain English timing instructions.
What about warning signs to watch for?
Surfaced prominently — when to rush back to hospital, when to call the doctor, what’s normal vs an emergency. Critical for post-surgery / post-cardiac / post-stroke discharges where deterioration signs need immediate action.
Will it explain insurance / billing items?
Discharge summaries are clinical, not financial. For the bill itself use Medical Bill Analyzer (separate tool) — it parses the itemised charges and surfaces IRDAI-reimbursable items.
What’s the credit cost?
3 credits per discharge. Cost is fixed regardless of length.
Privacy?
Discharge sent to inference provider for rewriting, not stored. For patient confidentiality (especially when sharing with extended family), redact patient name + UHID first via Redact PDF before running the simplifier.

Have lab reports too?

Blood Test Report Parser extracts structured lab values from any Indian lab format with high/low/normal flags. Pairs with the discharge summary for the full post-hospital handoff.
